Refer a Patient
To refer a patient, please fax the referral and available medical records to:

What to Send
When available, please include:
- Referral order and reason for consultation
- Recent office or hospital notes
- Relevant laboratory results
- Imaging reports
- Pathology reports
- Prior hematology or oncology records
- Current medication list
- Patient demographics and insurance information
