Privacy Practices

Your medical information is private, and protecting it is part of caring for you. This page explains how to get our Notice of Privacy Practices, what rights you have, and what this website collects.

Notice of Privacy Practices

Federal law requires us to protect the privacy of your health information, to give you a notice describing how that information may be used and shared, and to follow the terms of that notice.

You will receive our full Notice of Privacy Practices at your first visit, and you can ask for a copy at any time. Copies are kept at the front desk. If you would like one mailed to you, or you have a question about anything in it, call the office at (313) 791-7992.

Your Rights

When it comes to your health information, you have certain rights. In plain terms, you can:

  • Ask for a copy of your medical record, in paper or electronically.
  • Ask us to correct information in your record that you believe is wrong or incomplete.
  • Ask us to contact you a specific way, such as at a particular phone number, or to send mail to a different address.
  • Ask us to limit what we share about your care. We will say yes unless the law requires otherwise.
  • Ask for a list of the times we shared your health information and who we shared it with.
  • Ask for a paper copy of the Notice of Privacy Practices at any time, even if you agreed to receive it electronically.
  • File a complaint if you believe your privacy rights were violated. We will not retaliate against you for filing one.

This is a plain-language summary written to be useful, not a substitute for the Notice of Privacy Practices itself. Where the two differ, the Notice governs.

Privacy on This Website

This website is informational. There is no account to create and it is not a patient portal. You can read every page on it without giving us anything.

The only information we collect is what you choose to type into the contact form: your name, phone number, email address, reason for contact, and message. We use it to respond to you and nothing else. We do not sell it, rent it, or share it for advertising.

Please do not send health information through this website. The contact form is convenient, but it is not an encrypted or HIPAA-compliant channel. Anything clinical belongs in a phone call to the office or a conversation at your visit.

Tracking

This site sets no cookies. It runs no analytics, no advertising pixels, and no third-party tracking scripts of any kind. We do not know who visits, and we have not built anything here that would tell us.

One ordinary exception is worth naming: our hosting provider keeps standard server logs, the same as any website, which record addresses and requested pages for security and reliability purposes.

This site makes no third-party requests at all. Everything it loads, including the typefaces, is served from this domain, so no outside company is sent your address simply because you opened a page here.

Questions or Concerns

Direct any privacy question, records request, or complaint to our Privacy Officer. You will never be penalized or treated differently for raising one.
